Page one is a checklist of attached forms.

Page two is a checklist of the things you need to do in order to have a successful cleanup.

Page three is the 1020 on 10/20 Interest Sheet. Please fill this out and return it to Angelo Villagomez with Marianas Resource Conservation & Development Council ASAP. We need the information on this sheet so that we know where and when you are cleaning, how many people will be attending, if you need bags, and if there are any other activities that you will be participating in.

Page four and five is the USDA Earth Team Volunteer Signup sheet. Signing up as an Earth Team volunteer will provide you and your volunteers with insurance. Please turn this in before your cleanup. Fill out the first page and then if you need to sign up more than 8 people, please make copies of the second page.

Page six is the signup sheet for the day of your cleanup. THIS IS IMPORTANT: Please print enough copies of this for the day of your event. You can fit 20 names per page. We will only be able to varify that a volunteer participated in 1020 if they sign up on this sheet. PLEASE ENSURE THAT EVERYONE INVOLVED IN THE 1020 EVENT SIGNS THIS SHEET!!!!

The remaining pages are for MVA's Adopt-a-Road and DEQ's Adopt-a-Beach. We hope that after your cleanup you will adopt your area for the rest of the year. Please fill out and return these forms to either MVA or DEQ.
